The Cwmbran House Management Team has been shortlisted in the Outstanding Contribution Award category, while Joni Castle-Canavan, Hafod’s Operational Lead for Care, has been named a finalist for the Leader of the Future Award.
Both finalists have been recognised for their outstanding leadership and innovation, with both representing Wales in their respective categories.
The Leaders in Care Awards celebrate individuals and teams making an exceptional contribution to social care through leadership, innovation and excellence in care delivery.
That both have been shortlisted, reflects Hafod’s commitment to delivering person-centred care, supporting colleagues to thrive, and embracing innovative approaches that improve outcomes for residents.
Joni Castle-Canavan’s nomination recognises her remarkable career progression and significant contribution to care services. Since joining Hafod in 2015 as a Care Assistant, she has advanced through a number of leadership roles and now oversees seven care homes and home care services as Operational Lead for Care.
Over the past 12 months, Joni has led several transformational initiatives across Hafod’s care services. Her leadership is defined by a commitment to delivering exceptional care while fostering a positive workplace culture, recognising that valued colleagues are essential to achieving the best outcomes for residents.
Having progressed from frontline care through to senior leadership, Joni brings a deep understanding of the realities and challenges of care delivery. This experience enables her to balance operational excellence with compassion, ensuring the needs of residents, families and colleagues remain at the heart of every decision.
Speaking about her nomination, Joni said:
“I feel honoured to be finalist. Care is more than a profession for me, it’s something I’m deeply passionate about. Every day brings opportunities to make a difference, and that’s what has driven me throughout my career at Hafod.
“Looking back on my journey, I’m incredibly proud of what we’ve achieved together. This nomination isn’t just about me, it’s about the fantastic colleagues who go above and beyond for residents every day and make Hafod such a special place to be.”
The nomination for the Cwmbran House Management Team recognises the outstanding leadership of Home Manager Lynne Woodrow, Deputy Clinical Manager Alex Antonescu and Team Leader Leanne Moore.
Over the past year, the team has successfully led the home’s transition to nursing care while maintaining high standards of care and creating new opportunities for residents to thrive.
Their leadership has contributed to Cwmbran House achieving three “Excellent” ratings from Care Inspectorate Wales and establishing best practice that has since been shared across other Hafod care homes.
The team has also championed innovative, community-focused initiatives, including the introduction of Jolly Journey and a partnership with local nursery Meithrinfa Pontnewydd. The partnership brings residents and children together twice a week for intergenerational activities that promote wellbeing.
