This document sets out the minimum standards Suppliers need to meet to be considered further in the tendering activity, Suppliers must take note of any pass or fail questions. Suppliers can complete this document within the Qualification envelope on eTenderWales.
The Supplier Suitability Questionnaire is based on the https://www.gov.uk/government/publications/ppn-0323-standard-selection-questionnaire-sq.
For under threshold procurements the Supplier Questionnaire does not include grounds for discretinary or mandatory exclusion.

- Company Information
- Financial Information
- Company Policies
- Social Value
- Insurance Levels
- Accreditations
- Qualifications
Suppliers are asked to self certify and provide documentation as evidence if they move forward to the award stage. Suppliers must pass the qualification stage before they can move forward to the quality evaluation.
