Community care
We are committed to helping people stay in their own homes for as long as they choose and our highly trained and experienced team deliver a wide range of services to support this.
Services are tailored to individual needs and we can help with a range of tasks from personal care, medication administration, meal preparation, companionship and social trips to support with more complex clinical care.
Our reliable and responsive service enables us to help meet unforeseen emergencies and we can often step in to provide care which removes the need for hospital admission. We can also support faster discharge from hospital by delivering care needed at home which can help people recover faster, reducing anxiety for them and their families.
We are fully regulated by Care Inspectorate Wales (CIW) and our specialist team members offer free visits to discuss individual requirements and can advise on costs and any funding options available.
Continuity is a priority for the people we care for and their relatives. We ensure that they have a key point of contact in the office and a carer they are happy with. Our colleagues are up to date with all mandatory training and compliance checks as well as having access to additional training where appropriate.
What we do
Companionship – low confidence levels and feelings of loneliness or isolation are quite common following events such as illness or bereavement. The companionship of our care colleagues can help speed up recovery and help people to become re-established in their local community.
Personal care – we can assist with all aspects of personal care including washing, showering or bathing, dressing and grooming, getting up and going to bed, toileting and continence management, assistance with meal preparation and assistance with administration of medication where appropriate.
Complex care – we work with district nurse teams to enable delivery of more complex care tasks within the home, meaning that those being cared for can stay where they feel most comfortable.
Domestic help – we can assist with day-to-day tasks such as housework, laundry and shopping as well as maintenance services.
Social care – we can offer assistance with all aspects of community integration, including assistance with re-learning skills that may be required following a long period of hospitalisation such as eating out, using public transport or familiarisation with local facilities and amenities.
Night sitting – this is particularly useful to provide respite for carers or to allow people to be discharged from hospital at the right time. This can either be sleeping-in or waking nights where our colleagues are on hand throughout the small hours.
Escort duties – our qualified team members can escort and assist people in attending a range of activities including hospital appointments or on social trips.
We believe there are very few instances where a person would be unable to remain in their own home. The above are just examples of the services we can offer, please contact us to discuss your specific requirements.
We are a preferred supplier for several local authorities in south Wales.
